یک سیستم پایش موقعیت بیمار به سیستم دروازه بندی تنفسی اضافه شده است که برای هم زمان کردن تصویر برداری و پرتودرمانی در سیکل تنفسی بیمار به کار می رود .این سیستم جدید هر گونه حرکتی را که بر صحت درمان تاثیر می گذارد آشکار می کند . شرکت سیستمهای پزشکی ( Varian (USA ) اخیراًً مجوز K) 510را ازFDA برای سیستم دروازه بندی تنفس RPM دریافت کرده است .
به گزارش سایت جامع علوم پزشکی پرتو ایران (WWW.PRIN.ir ) وبه نقل از Medinews.com ، سیستم RPM از طریق قرار گرفتن یک جعبه نشانگر منعکس کننده بر روی سینه یا شکم بیمار و پایش حرکت آن بوسیله دوربیتهای مخصوص که در اتاق درمان قرار گرفته اند ، کار می کند . سیستم قبلی حرکت بلوک نشانگر را فقط در راستای عمودی پایش می کرد تا آن را با حرکات تکرار شونده و ریتمیک تومور که هنگام تنفس بیمار رخ می دهد مربوط سازد و بوسیله آن دسته پرتو را بسته به اینکه تومور درون میدان قرار دارد یا خیر ، روشن و خاموش کند .
سری جدید سیستم RPM حرکت بلوک نشانگر را در تمامی سه جهت بررسی می کند ، یعنی به صورت بالا و پایین ، جلو وعقب و پهلو به پهلو در صورتیکه بیمار حرکت کرده و نیاز به وضعیت دهی مجدد باشد ، نمایشگر جدید بر روی کنسول بلافاصله تکنسین را آگاه می سازد .Mika Mietlinen مدیر فروش سیستم RPM می گوید : این سیستم در مورد هر بیمار کاملاً حرکات را کنترل می کند و نشان می دهد که چگونه بلوک در اثر حرکات بیمار ، حرکت می کند . اگر بیمار بر روی تخت درمان در هر جهتی بیش از حد طبیعی حرکت کند ، سیستم بلافاصله هشدار داده تا درمان متوقف شده و بیمار وضعیت دهی شود.
Richard stark ، مدیر فروش جهانی Varian می افزاید : برای دستیابی به نتایج بهتر حین درمان بیماریهایی مثل سرطان ریه ، پزشکان باید حرکت تومور را پایش کرده و آن را حین درمان در نظر داشته باشند . با ترکیب تصویر برداری اشعه X در طول درمان با تصویر برداری نوری که حرکت تومور را دنبال می کند ، پزشکان به طور موثری تومور را بی حرکت می کنند و بافت سالم کمتری را در معرض تشعشع قرار می دهند.
Mietlinen می گوید : بی حرکت سازی ، به خصوص برای تومورهای متحرک موجود در قفسه سینه هیچ گاه کامل نیست . نمی توان بیماران را کاملاًً ثابت کرد و آنها برای کمی حرکت آزادی دارند . با سری جدید سیستم دروازه بندی RPM ، اگر بیماری حرکت کند ، پزشک متوجه خواهد شد.

Technology Monitors Patient’s Position During Radiotherapy, Radiosurgery

New patient position monitoring capabilities have been added to a respiratory gating system, which is used to synchronize imaging and radiation therapy treatment with a patient’s respiratory cycle. The new feature detects any motion that compromises the accuracy of the treatment.
Varian Medical Systems, Inc. (Palo Alto, CA, USA) recently received U.S. Food and Drug Administration (FDA) 510(k) clearance for its RPM respiratory gating system.
The RPM system works by placing a reflective marker box on the patient’s chest or abdomen and monitoring its motion using special cameras positioned in the treatment room. The previous system monitored the motion of the marker block only in the vertical dimension, to correlate it with the rhythmically repeating tumor motion that occurs as the patient breathes, and to trigger the treatment beam on and off as the tumor moves in and out of range. The new version of the RPM system analyzes the marker block motion in all three dimensions as it moves up and down, forward and back, and side to side. A new display on the system’s console immediately alerts the radiation therapist if the patient moves and should be repositioned.
“The system actually learns, for each patient, how the marker block is going to move when that particular patient stays still and breathes normally,” said Mika Miettinen, product manager for the RPM system. “If the patient shifts on the treatment couch in any direction beyond the natural range of motion, the system alerts the clinician to halt the treatment and reposition the patient.”
“To achieve better results when treating diseases such as lung cancer, clinicians must be able to monitor the motion of the targeted tumor and to account for that motion during treatment,” added Richard Stark, Varian’s vice president for worldwide marketing. “By combining x-ray imaging during treatment with optical imaging that tracks tumor motion, clinicians effectively ‘freeze’ the motion, exposing less healthy tissue to the treatment beam.”
“Immobilization--especially for moving tumors in the thorax--is never perfect. Patients are not locked down; they have some freedom to move,” Mr. Miettinen added. “With this new version of the RPM gating system, if a patient moves, the therapist will know it.”
